This endpoint finds a fuel surcharge table by name. Each table comes with its id, name, mode, fuelPricesId
and fuelSurchargeCalculatorId; isDefault is always false. mode: 0 FixedPercentage, 1 FixedPricePerDistance,
2 CalculatedPercentage, 3 CalculatedPricePerDistance. The rates per vehicle type in force on a date are
returned by GET api/v1/fuelsurcharges/tables/{id}/fuel-surcharges/active. Requires an administrator; other
users get a 403.
